IMPORTANT
The parents are advised to note their
details mentioned in the particulars column of the Admission Form filled by
them.
If any changes are required in the
admission details, the following documents are required to be submitted in the
school office.
In case you want to make
change in name:
1) Current date application.
2) Original affidavit reflecting name change.
3) Original copy of nationalized Hindi and English newspaper.
4) Copy of Gazette.
5) Self attested copy
of identity proof.
In
case you want to make change in Date of Birth of the student:
1) Current date application stating the reasons
of recording the wrong entry of Date of Birth in the child’s existing personal
details.
2) Original affidavit on Non-judicial stamp paper worth Rs.10/-(Rupees
Ten Only) with an undertaking attested by First Class Magistrate as per the
format.
3) Copy of certificates if any received from school
reflecting Date of Birth.
4) Self attested copy of identity proof i.e. Voter ID or
Aadhar Card of either parent establishing relationship with child are required.
Note:
Please report in the school office if you observe any discrepancy which is not
as per the record filled by you in the particulars column of the Admission Form.
